Sometimes support includes more than conversation. For established therapy clients, clinical documentation may be considered when it is connected to your care and supported by an appropriate assessment.

Letters and forms are not one-size-fits-all. We take time to understand your symptoms, daily functioning, treatment needs, and the purpose of the request before deciding whether documentation is clinically appropriate.
This process protects the integrity of your care and helps ensure that any information provided is accurate, relevant, and within the therapist’s professional scope.
What to expect
A careful process, explained clearly
Establish care
Documentation is available only to therapy clients after a sufficient period of assessment, with a minimum of three sessions.
Review the request
We discuss the purpose, deadlines, requirements, and how the request relates to your treatment.
Determine appropriateness
Completing three sessions does not guarantee documentation. Any letter or form must be clinically supported and within professional scope.
Fees & timing
Discussed before work begins
Documentation is separate from therapy services. Fees depend on the type and complexity of the request and will be reviewed with you before any paperwork is completed.
Because outside organizations set their own requirements, completion of documentation cannot guarantee that a request will be accepted or approved.
